May 3, 2008 Entry Fee: $35 for Amputee, $40 for Guest Amputees are encouraged to bring a guest and will be paired with 2 PGM students. If you don't have a guest, we'll fill out the foursomes. Entry fee includes golf, cart, food (before & after), beverages, and various proximities for special events. There will also be a raffle for golf merchandise donated by area sales reps. Format of Play: Scramble; 8:30 shotgun. Each team will consist of an amputee player, guest, and 2 PGM students. Confirmation and directions will be sent to each entrant. Hotel reservations can be made at the following local hotels:  Country Inn (231) 527-9000, Holiday Inn (231) 796-4400, Best Western (231) 592-5150. You are invited to come up the day before and play a practice round. Please call Katke Golf Course at 231-591-3765 for reservations. Practice round fee will be $25 and includes cart.
